Terms and Conditions:
Cash Rebate Offer good on purchases made at authorized retailers between November 1, 2009 and December 31, 2009. Minimum total retail purchase requirements must be met excluding non-Omix family brand products that may be included in the retail invoice/receipt, tax, freight or any other charges. All requests must be postmarked no later than February 1, 2010. PO boxes will not be accepted. Maximum rebate for any one consumer and address is $200 during this 2 month period. Only one rebate per household. Allow up to 6-8 weeks for processing. Original label containing the UPC code (found on the outside of the product's package) and an original receipt or invoice are required. Rebate checks will be paid in U.S. Dollars. Cash redemption value is 1/100 of 1 cent. Keep copies of all materials submitted; originals will not be returned. Not responsible for lost, late, damaged, misdirected, incomplete, incorrect, or illegible requests. Offer is limited time only. False information or misrepresentation disqualifies this claim and violators may be prosecuted. Offer void where prohibited, taxed or restricted by law.
